It is best to introduce decoherence with a simple example.
Before doing so, we need to have something to decohere.
The job of decoherence is to bring a quantum system into an apparently classical state, (trams, cups of tea, bread and jam,what we view and experience and believe to be the real state of things, our 'classical; world). Hard for our 'classical' minds which we all have to understand this - how something can be in two, or many places at the same time
However what underlies our classical world is the superposition (things in two places at the same time) or a superpostiion of states.
Yet when we intervene in the quantum state by an act of intervention, say by measuring
we collapse the quantum state into the classical state,
We decohere the quantum state into a classical state.
